Vision Screening Notification

Parents/Guardians,

In accordance with Utah state vision screening requirements, vision screenings will be conducted for 7th and 9th grade students beginning September 8th.  Screenings will take place during students' 2nd period classes over approximately 2-3 weeks.  

Vision screening helps identify students who may have reduced vision or signs of a possible vision concern.  Screening does not replace a comprehensive eye examination by an eye care professional.

If your student wears glasses or contact lenses, please remind them to wear them during the screening period.

Parent's/guardians may opt their student out of vision screening. If you do not want your student to participate, please complete the Vision Screening Opt-Out Form  Completed forms may be emailed to pstaley@murrayschools.org or returned to the school office. The opt-out must be submitted annually in writing.

No action is required if you would like your student to participate in vision screening.
